Re-issue of Passport in lieu of lost or damaged passport - Adults - 36 Pages

Re-issue of passport is required when an Indian passport is lost or damaged. In line with International Civil Aviation Organisation (ICAO) regulations, only machine-readable printed passports are accepted for international travel, and short validity or emergency passports are no longer issued in such cases. Applicants must apply for a fresh passport through the normal re-issue process.

In situations of extreme emergency, such as serious illness or death of an immediate family member, a one-way Emergency Certificate may be issued to facilitate urgent travel to India. For any further travel, a new passport must be obtained from the nearest Passport Issuing Authority in India.

Note: Old passport shall be cancelled by the High Commission and cannot be used for travel once application for reissue is accepted.

Follow the below steps in order to apply

  1. Create an account (Register yourself) on Alankit Indian Consular Application Centre (ICAC) website. Click Here to register.
  2. Fill the Govt of India's Online Passport Application Form, by following these steps:
    1. Register through the Global Passport Seva Online Portal. (Choose your country of residence, complete the required information and click on "Register" link. Click Here to visit the Global Passport Seva Online Portal.
    2. Login to the Global Passport Seva Online Portal with the Login ID created in the previous step.
    3. Select "Type of Service"
    4. Fill in the required details in the application form and submit.
    5. Click the "Print Application Form" link to print the application details containing Application Reference Number (ARN) and other details.
  3. Book your appointment online at Alankit Indian Consular Application Centre (ICAC) website. You can choose a convenient date and time to visit the Alankit ICAC for your application submission. Click Here to book your appointment online.  
  4. Ensure that you have your passport, photographs and all other supporting documents, exactly as per the "Documents Checklist" section. 
  5. Visit the Alankit ICAC, as per your jurisdiction. The address and location of the ICAC will be clearly mentioned in the appointment letter. 
  6. Kindly ensure you reach the ICAC 15 minutes prior to your appointment time. 
  7. Submit your application and deposit your fees at Alankit ICAC. You can check the fee for this application under the "Fee Details" section. 
  8. Enroll for Biometrics at Alankit ICAC (if required)
  9. Your application submission process is now complete! You can avail our Courier service to get your passport delivered to your doorstep!

Document Checklist

  1. Kindly complete the online passport application form by registering through Government official portal. Download and print the form, and sign on the first and last pages. Click here for application form.
  2. Ensure all details are correctly filled as handwritten corrections are not permitted, and all entries must be clearly visible after printing.
  3. In case of abbreviated names, please provide the full expanded name in the application along with valid documentary proof.
  4. If applying for a complete name change or after marriage/divorce, kindly fill the form using the name you wish to have on your new passport.
  5. Ensure you provide your current landline and mobile numbers correctly to enable easy communication if additional information is needed.
  6. Kindly provide your Indian address in the respective fields of the application as it is required for police verification.
  7. You need to provide two recent passport-sized photographs (2 inch x 2 inch) taken within the last six months, following the official photo specifications. Click here.
  8. Kindly submit your original current passport (not applicable in cases of lost passport).
  9. Please submit the duly filled and signed Personal Particular Form. Click here.
  10. Submit your current visa status proof, such as VEVO, Visa Grant Notice, or visa sticker, and for bridging visas, include both VEVO (not older than 30 days) and Visa Grant Notice clearly mentioning the visa category.
  11. As per Alankit, kindly make the payment of applicable fees through EFTPOS, MasterCard, Visa Card (issued in Australia), cash, money order, or postal order payable to the authorized centre; personal and company cheques are not accepted.
  12. Now, visit an Alankit ICAC Centre in person with all the required documents. Make sure to schedule an appointment in advance and arrive at least 15 minutes early.

Additional Documents Based on Specific Conditions

  1. If your passport expired more than one month ago, kindly submit Form I attested by JP or Notary (NSW residents to use Notary only), a JP-attested copy of Australian address proof, and copies of two Indian photographic IDs such as driving license, voter ID, or Aadhaar card.
  2. If your current signature differs from the one on your passport, please provide a statutory declaration attested by JP or Notary (NSW residents to use Notary only) stating that both signatures belong to you.
  3. If you wish to add, delete, or change your spouse's name in your passport or change your surname after marriage/divorce, kindly submit relevant marriage certificate, divorce decree, death certificate, or joint photo declaration, as applicable.
  4. If you need to change the order of your name or split your name in your new passport, kindly submit a self-declaration mentioning the correct name format.
  5. If your appearance has considerably changed, such as from with a turban to without, kindly attach Form J attested by JP or Notary (NSW residents to use Notary only) along with a recent photo.
  6. If you wish to update your address in your passport, kindly submit a JP or Notary attested copy of a utility bill reflecting your new address, noting that if you are not a permanent resident in Australia, an Australian address is not acceptable.
  7. If you are renewing your passport for the first time after attaining 18 years of age, kindly attach a copy of your educational certificate, such as your 10th-grade pass certificate, to confirm your Non-ECR category.
  8. If you require a complete name change or wish to revert to your maiden name when not divorced or widowed, kindly submit copies of two Australian national newspapers or gazette notification from India and two public or school documents issued in your desired name; permanent residents may submit two documents issued by Australian authorities, while others must provide documents issued by Indian authorities.
  9. In case of lost or damaged passport, please submit Form V attested by Notary (NSW residents to use Notary only), a copy of the FIR or police report including your full name, date, and place of loss (only event numbers are not acceptable), copies of the front and back pages of the damaged passport, reference letters from two Indian nationals residing in Australia with self-attested copies of their Indian passports (both front and back pages), and documents supporting your personal details such as name, date of birth, and place of birth as mentioned in your passport.

Bank Transfer Charges

FAQ's for Passport Services

  1. When do I need to apply for re-issue of my passport?
    • You must apply for a re-issue if your Indian passport is lost or damaged, as emergency or short validity passports are no longer issued.
  2. Is police verification mandatory?
    • Yes, you must provide your Indian address in the application, which is required for police verification.
  3. What is Form V and why is it required?
    • Form V is a declaration for lost/damaged passports. It must be attested by a Notary Public (NSW residents must use a Notary only). This form certifies the circumstances of the loss or damage and is mandatory for processing your application.
  4. What if I don’t have copies of the lost passport?
    • If you cannot provide any copies of your lost passport, ensure your other submitted documents (such as educational certificates, birth certificate, or Aadhaar) accurately reflect your name, date of birth, and place of birth as recorded in the lost passport.
  5. Can I use my current Australian address in the passport application?
    • Only if you are a permanent resident. Otherwise, provide an Indian address for police verification.
